Partner in Care Spotlight: Sabrina Carter, FNP-BC and Positive Pathways

Our Partner in Care series highlights the dedicated individuals and organizations we work alongside to ensure that every person in our community has access to meaningful support. This month, we're proud to spotlight Sabrina Carter, RN, BSN, FNP-BC, founder of Positive Pathways, a transformative mental health practice based in Price, Utah.

A Positive Vision for Rural Mental Health

Founded in 2017, Positive Pathways is more than a clinic—it’s a mission in motion. Sabrina returned to her hometown after 20 years away, motivated by a deep awareness of the gaps in mental health services she witnessed growing up—and a desire to change that reality for future generations.

“I grew up without access to services, and I was shocked to see that nothing had changed,” she shared. “I knew I could make a difference by bringing my expertise home to serve the people and place I love.”

Positive Pathways operates with two guiding goals: to shift cultural attitudes toward mental health through education and time, and to reduce treatment disparities in Carbon, Emery, and Grand Counties. With a whole-person, evidence-based approach to care, Sabrina and her team focus on both the mind and body—offering individualized treatment plans that promote hope, resilience, and recovery.

Meeting Challenges with Compassion

The challenges rural communities face when it comes to mental wellness are complex—limited providers, transportation barriers, stigma, and a lack of accessible crisis resources. Sabrina has seen these realities firsthand and built Positive Pathways to be part of the solution.

“When we started, local therapists had one- to two-month waiting lists. We knew we had to do things differently. Healing takes time, collaboration, and a willingness to meet people where they are—physically and emotionally.”

While privacy laws prevent Sabrina from sharing specific patient stories, she notes that some of the most rewarding moments in her career have come from supporting individuals through recovery and sobriety.

“It’s far more rewarding than I ever imagined. Watching someone reclaim their life—there’s nothing more powerful.”

A Powerful Partnership

One key piece of the Positive Pathways model is collaboration—and the partnership with Four Corners has helped extend care to even more people across the region.

“Our work with Four Corners Behavioral Health has allowed us to serve populations we may not have reached otherwise. It also gives people the opportunity to choose how and where they receive their care—which is empowering.”

Sabrina believes that collaboration is essential at every level of care—from provider-client relationships to cross-agency partnerships.

“Transformation happens through partnership. Improving health outcomes in our state isn’t possible without working together.”

Looking Ahead: Innovation and Inspiration

Sabrina is excited about the potential of technology to expand mental health services across rural Utah. From telehealth visits to online education, she sees great opportunity in meeting people where they are—especially in communities with limited in-person resources.

More than anything, she wants people to know that mental health care isn’t just about crisis—it’s about living well.

“We’re not just here to hand out prescriptions or manage problems. We’re here to partner with individuals to help them thrive, discover their strengths, and build a foundation for lifelong wellness.”

Words of Wisdom

When asked what keeps her going on the hard days, Sabrina points to nature—and the importance of self-care.

“There’s something about Carbon County sunsets and mountain views that remind me healing is always possible. I also take a lot of vacations—because you can’t pour from an empty cup.”

Her advice to others looking to make a difference?

“Step outside your comfort zone. Be willing to fail—that’s how you grow. Listen to your community. Find people who don’t see you as competition. Find your collaborators. And remember: every small act of care creates ripples far beyond what you can see.”
