The Mission of Four Corners Community Behavioral Health, Inc. is to offer highly responsive and effective mental health and substance abuse services to the residents and communities of Carbon, Emery and Grand Counties.

As the Primary Mental Health Provider for our three counties, we partner with numerous agencies for outpatient, inpatient and residential treatment. Prior authorization is required for all services not provided by Four Corners Behavioral Health.
